- Groups with 2-3 eligible employees: Two employees must be in CA
- Groups with 4 eligible employees: Two eligible employees may be located out of state
- Groups with 5-99 eligible employees: Two employees must be in CA
CoPower ONE Voluntary PPO plans:
- Groups with 5-99 eligible employees: Two employees must be in CA
CoPower ONE HMO (Good, Better, Best, and Voluntary) plans:
- Service must be rendered in CA
